W-9
The W-9, also known as "Request for Taxpayer Identification and Certification Form," is used to verify a persons TIN, or taxpayer identification number. Employers are required to file, with the IRS, and information return and must obtain the correct TIN to report income paid, transactions, mortagage interest paid, etc.
FILLING OUT A W-9 FORM - INSTRUCTIONS & TIPS
1. If you are designated as a "foriegn" person, you are not required to use a W-9 form; a W-8 must be submitted.
2. Employers are prohibited from disclosing any employee's personal information. If it is found that employers were negligent, the organization may be subject to civil and criminal prosecution.
3. Taxpayers must supply employer identification number, SSN, or others identifiers to a payor so proper reporting can be done to the IRS. If information is lacking, the payor withholds some of the payment.
